Document Footer

You should have the following elements at the bottom of documents (sometimes called footers). Again, these elements should be similarly positioned in all documents:

The author of the document
Author contact information, including email address
Last modification date of document
A hyperlink to a top-level or home page
Copyright status of document (if applicable)
Links to related documents (if applicable)

Here is a sample footer:
