This probably isn't a very good illustration of the incentive mechanism for any purpose. Obviously no amount of work would stave off disease. Also, the incentive scenario is normally used to point out that the hardest workers would slack off because they would only get what everyone else gets, not that people who are "born lazy" just plain won't work at all. (Even if it means death.)
