Actually it is a very tight fit so that it will never fall off. Provided you
have it oriented correctly (see the 'Alice' & 'Lisa' arrows on the top of
the adapter) you need to give it a real good push down with both thumbs
until it solidly clicks int place.

Yes it does look as if you had exactly the same problem - you hadn't pushed
it on hard enough.
