Sorry to disappoint you but that's an old one. I first documented it in the
Magnum Opus Guide V1.0 back in 1996. AFAIK it's been in every version of
DOpus since 5.0.
This requester only appears after several attempts and Cancel's of quitting,
sometime it can take more attempts than others to appear. Took me a while to
get a screen grab of it.
