Hi,
I installed a Mediator PCI 1200 board and a Voodoo 3
graphics card into the Amiga. I've checked and double
checked all cables etc. Everything seems to be OK
until I boot. I get the message:

PFS-3+ Error Requester
Initialisation Failure
OK
-
When I click OK the Amiga boots to a blank screen.
That's it...
I have also disconnected the Mediator and V3 and the
Amiga works fine.
Any ideas?
My setup is as follows:
A1200T, OS3.9, PFS3, Apollo 1240/25, 2+32Mb RAM, Power
Flyer Gold, Scan Double/Flicker Fixer, 2.5 IDE & 3.5
SCSI HD, 8x CD-ROM
Jumper settings:
Mem Win = 8Mb
Config = Not used
Wait = ON
