If you don't need too many of them, you could always make your own ;^)
Assuming you've got a printer that'll take thin card, I can let you have a
DrawStudio file (if you've got DS) or a PostScript file (if you've got
GhostScript or TurboPrint 7) with a suitable layout
All I've got to do now is knock up a template for 80mm CDR sleeves ;^)
