I've seen this occur when you snapshot and there is something which is
'special' (E.G. non-disk or wierd), such as leaving a CD-ROM in the tray, as
the CDROM is not writable, it goes to the top of the heap. (Note that this
is my experience with IDEFix and CacheCDFS as well as CacheCDFS and a SCSI
CDROM.
Sometimes, I've seen that a mixture of different sets of icons can also
cause this (NewIcons and GlowIcons at the same time), assuming that your
custom libraries/binaries for support of these may have become hosed.
