I ran TitleShadow right after Setpatch and Env-Handler much further down,
just before the system assigns! Even after removing Env-Handler the problem
of asking for 'Volume Env' to be inserted still persisted! All the ENV setup
commands and ENVARC copy commands where there but it still asked for 'Volume
ENV' to be inserted! Cancelling the requestor allowed workbench to boot but
TitleShadow was crap until saved. After reboot I was back to square one
again!
Removing TitleShadow cured the problem.

I've now placed TitleShadow as the last line of my user startup and put
Env-Handler back before the system assigns and all works well!
My opinion is that TitleShadow MUST be run in the user startup NOT in the
startup sequence after Setpatch as the documents suggest!
