It sounds as if there are physically bad blocks on the disk, rather than a
problem with the File System. When it tries to read from the bad area, it
gets confused and barfs. No amount of formatting will repair a physically
damaged block.
Have you tried mapping the faulty blocks with something like HDInstTools?
This will build a list of the bad areas and tell the drive not to use them.
Once the drive knows where the bad blocks are, you should be able to format
it.
