Same here, I always type it in exactly as it appears, MR G E GRIFFITHS in
this case.

I'd suggest United Kingdom as that seems to be the way most sites on the
'net offer.

Zip code is the US equivalent of a postcode, so put it in there. Don't put
anything in State unless it says to put "N/A" or something. I usually put a
space in my post code when ordering from the 'net but it shouldn't really
matter.
