Create an Outlook form
Note All custom forms must be based on existing Outlook forms. Also, disable Microsoft Word as your e-mail editor while you're designing forms; otherwise, the forms design environment won't be available from the form. If you do not wish to use the default form functionality provided by Outlook, you can hide all of the default form pages and use only the extra blank pages to create a complete custom form.

To begin designing an Outlook form:
- Open an Outlook item. This can be accomplished by various methods. For example, to open a new mail message item, you can:
- Click on the New Mail Message icon to open a mail message.
- Click on the arrow beside the New Mail Message icon and click on Mail Message.
- Select New on the File menu and click on Mail Message.
- Select New Mail Message on the Actions menu.
- Double-click anywhere in an empty area of the Inbox view pane.
- When you have the new form opened, on the Tools menu of the form, select Forms and click Design This Form. You may also click Design Form and choose to design a different form.
- Add the fields, controls, and code that you want to your new form.
