THE MAIN MODULE: LEHIPEDS
As previously noted the Main Module handles the two major files in
the LEHIPEDS program, the Master and Service files.
<< MASTER FORM >>
We will first look at the Master Form in some detail. This is the
form that creates records for the Master File. If you have not
already done so, you should read the general discussion of forms
in the preceding section of this Manual.
Review the Master Form in the program. The data to be entered into
each field is pretty much self-explanatory.
<< REQUIRED FIELDS: MASTER FORM >>
Some fields are "required fields", i.e. you are not allowed to
leave them blank. The required fields are:
Dr: enter Dr.name or press F5 for list of doctors
First Name: enter first name
Last Name: enter last name
Male/Female: enter X (in either field)
DOB: enter in required format
Acct Type: enter or press F5 for list of acct types
<< POP-UP "MINI-TABLES" FOR DATA ENTRY >> For Dr. or Acct Type,
pressing F5 will pop-up a mini-table of possible entries. Use the
Up or Down Arrow_keys to highlight the desired entry and press the
Enter_key. Alternatively, you may type in a different entry. The
City and Zip fields also allow F5 to pop-up a list of possible
entries, although these fields are not required fields. The
pop-up tables are "programmed" in the Custom Module; more about
this later.
<< ID_NO FIELD: MASTER FORM >>

The field, ID_No., is a "display field". The field is not intended
to accept an entry; just press the Enter_key to get past it.
All patients must have an ID_No. How then do they get one? From
the scrolling table press the Ins_key to call up a new Master
Form. Note that the ID_No. field automatically increments to the
next available ID_No. This would be one higher than the last
ID_No. in the database.
What happens if you try to type in an ID_No that is currently in
use. The program will not let you do this. You get a beep and the
status field reads "Creates Duplicate Entry" (We didn't tell you
about that message.)

What about records that have been deleted? Can you re-use those ID
numbers, by overwriting the "auto-incremented ID_No." in a new
patient Master Form? The answer is "yes", but it's not worth the
effort to keep track of all those deleted numbers. Let the deleted
ID numbers be lost in cyberspace; the program supports 99,999
Master Forms. There will be an upgrade long before you're ready to
enter patient No. 100,000!

<< INS_INFO FIELD: MASTER FORM >>
The field, Ins Info, is for whatever insurance information, if
any, you may wish to enter. You may want to put in here deductible
amounts or other restrictions.
<< PROF_DISC FIELD: MASTER FORM >>
The field, Prof Disc, allows entry of a whole number representing
a percent professional discount. Of course, you may leave this
field blank. If you do specify a percentage, said percentage will
automatically be deducted from your total charges when doing an
Encounter Form.
<< C-STATUS FIELD: MASTER FORM >>
The field, C-Status, is also a display field. In this case the
cursor doesn't even stop in the field. This field displays the
patient's collection status; every patient starts out with a
default "OK" in this field. The collection status may be modified
to C1, C2, or C3 in the ledger section of the Main Module.
<< UPDATED BALANCE: MASTER FORM>>
Note the field, Updated Balance, in the lower right-hand corner.
This shows the patient's outstanding balance at the present time.
This is a "stored balance" as opposed to a "calculated balance".
The stored balance comes from calculations from the last
Encounter Form for the patient. It is usually accurate although
sometimes it may be incorrect if there have been recent multiple
transactions for the patient. You can check a calculated balance
(which is based on calculations from all Encounter Forms, and is
therefore always correct), by pressing the F10_key.

TIP: Checking a calculated balance from the Master Form: Press the
F10_key. A small window will pop-up on the right side of the
screen advising you to press the Enter_key to get the calculated
balance. The balance will be calculated in the pop-up window. Then
press the Esc_key twice. This will place the calculated balance in
the lower right hand corner "display field" of the Master Form,
and will clear the pop-up window.

TIP (continued): If the calculated balance was different, don't
forget to save the form (by pressing Enter from last field or
Ctrl-Enter from any field) so the correct balance now showing in
the display field will be saved.
Checking the balance from the Master Form comes in handy. If you
pull up the Master Form for a patient (or parent) calling for an
appointment, you can quote any balance they may owe at present.
It's always a good idea to verify the stored balance by checking
a calculated balance with the F10_key.

KEY POINT: It is important to remember that this program does NOT
create any ledgers or statements based on stored balances. The
ledgers are always based on calculated balances and are therefore
always accurate.

<< ENCOUNTER FORM >>
We now turn to a detailed look at the Encounter Form. Again, be
sure to read or reread that section of the manual which discusses
forms in general.
The Encounter Form, as the name implies, records a patient
activity or "encounter". The encounter may involve an office
visit, a hospital admission, payment of a bill, or whatever. In
the case of a medical service, several different services may be
included on a single Encounter Form. The Encounter Form may
include an office visit, lab work and an injection. In the case of
a hospital medical service, a single Encounter Form might include
a hospital admission, 2 hospital daily visits, and a hospital
discharge.
From the Main Menu highlight and select "Patient Encounter Forms".
From the next submenu select "Create/Edit/Delete Encounter Form".
This will give you a scrolling table of patients in the database.

KEY POINT: Most likely this is a screen that one of your computers
will be "sitting at" once the day's office business has started.
This is where you will select any patient that needs an encounter
recorded.
Obviously, in the case of a new patient, you will first need to
create a Master Form so that he or she will show-up in the
database.

Highlight the desired patient and press the Enter_key. One of two
things will happen:
- If the patient has had previous encounters, another table
will appear listing those encounters. From this table you
will press the:
Ins_key to create a new Encounter Form
Enter_key to modify or review an existing Encounter Form
Del_key to delete an existing Encounter Form
- If the patient has had no previous encounters, it is assumed
that you want to create an Encounter Form. Accordingly you
are taken to a blank Encounter Form headed up for that
patient.
Review an Encounter Form for a patient. Note that you can get help
by pressing F1. Also note that use of the various function keys
are highlighted in the lower right had corner of the form.
<< DISPLAY FIELDS: ENCOUNTER FORM >>
The first line below the status field contains a group of "display
fields". These fields identify the patient and the encounter and
require no data entry. The patient ID_No, Name, and birthdate are
displayed. Also the Enc_No is displayed.
<< ENC_NO FIELD: ENCOUNTER FORM >>
The Encounter Number is very important because it uniquely
identifies a particular encounter. It may be necessary to later
track the encounter using this number. Note that in creating a new
Encounter Form the program automatically increments the Enc_No to
the next available number. When an Encounter Form is deleted, the
Enc_No is "lost" and never reused. The program supports 999,999
encounters: an upgrade will be available long before you get to
your millionth encounter.

<< RPT_DATE FIELD: ENCOUNTER FORM >>
This is an important "required field", that is, the program does
not allow you to leave it blank. The "Report Date" is the date
that you are actually filling out the Encounter Form. Two
important points follow:
- The Report Date does not necessarily have to be the same as
the date services were rendered. The Report Date and Date of
Service may be the same in the case of an office visit.
However, the Report Date and Date of Service may be different
in the case of hospital services.
- The Report Date is the date the charges and/or payments
entered on the Encounter Form will be reflected in your
business summary. The daily business summary will include all
Encounter Forms with the same Report Date regardless of the
date medical services were provided.
<< WHERE FIELD: ENCOUNTER FORM >>
This field allows one of two entries, "OFF" or "HOS", to indicate
where medical services were provided. This is a required field.
Select OFF or HOS by pressing the Right or Left Arrow_key.
<< CLAIM TYPE FIELD: ENCOUNTER FORM >>
You may enter the claim type in two ways: 1) type it in, or 2)
press the F5_key to get a pop-up table of available claim types.
It is suggested you use the second method to avoid misspelling the
claim type. Claim types in the pop-up table are user-defined in
the Customization Module.
Usually the claim type will be the same as the patient account
type which was defined in the Master file. However, there are
exceptions. For example, a Medicaid patient may have a Medicaid
claim type for an illness visit, and an EPSDT claim type for a
health check visit.
Note the reminder (in inverse video) regarding the F5_key, as well
as other function keys, in the lower right hand corner of the
Encounter Form.
<< DR FIELD: ENCOUNTER FORM >>
Enter the code for the doctor providing the medical service by 1)
typing it in, or 2) pressing the F6_key to get a pop-up table of
available doctor codes. Doctor codes, which can be up to 5
characters, are user-defined in the Customization Module.

<< DX and CD FIELDS: ENCOUNTER FORM >>
The Encounter Form includes 4 sets of fields to enter diagnoses
and appropriate diagnosis codes. You may use anywhere between none
or all of these fields. If desired, you may individually type data
into each field.
There is a much easier way. From a diagnosis field press the
F9_key. This will pop-up a table of available diagnoses and codes.
Highlight the desired diagnosis and press the Enter_key. This will
put the diagnosis and code into the appropriate fields of the
Encounter Form. The cursor will be advanced to the next diagnosis
field. To enter another diagnosis/code set, repeat the process by
pressing the F9_key.
The diagnosis and code pop-up table is user-defined in the
Customization Module.
<< DATE_SRV FIELD: ENCOUNTER FORM >>
This is a required field which specifies the date of medical
service. There is only one Date_Srv field for all 8 lines on the
Encounter Form. This is appropriate for office visits since all
the services would be provided on the same day.
What about hospitalizations where you have more than one date of
service? On line 1 enter the first date of service in the Date_Srv
field followed by a service description. On lines 2 through 5
enter any additional services for the first day and services for
subsequent days. The additional dates of service are entered when
you complete the Insurance Form in the Insurance Module.
Note that we did not advise using lines 7 and 8 for hospital
billing. This is because the HCFA 1500 Insurance Form only
supports 6 lines of service.

<< CPT, MOD, SERVICE, CHARGE FIELDS: ENCOUNTER FORM >>
These fields are for specifying CPT codes, service description,
and charges for the medical services provided. They are repeated
in 8 lines on the Encounter Form.
You may enter these data by individually typing in each field.
There is a much easier way to handle these entries, similar to the
entry of diagnosis and diagnosis codes discussed above.
From the CPTcode field press the F10_key to pop-up a table of
available codes, descriptions, and charges for the various
services that you provide. Highlight the desired selection and
press the Enter_key. This will place entries in the CPT code,
modifier (if any), service description, and charge fields; the
cursor will be placed at the beginning of the next line. Repeat
the process if needed.
The pop-up table is user-defined in the Customization module.
What if you only have entries on one or a few lines? How do you
get to the end of the form? Do you have to press the Enter_key
numerous times to go through all the fields in the remaining 8
lines? You can do this if you want, but there is an easier way.
Note the instructions for the various function keys in the lower
right hand corner of the Encounter Form. Pressing the F3_key takes
you to the Amount Paid field from anywhere on the form. This
allows you to bypass the remaining lines that you intend to leave
blank.
<< FINANCIAL FIELDS: ENCOUNTER FORM >>
These are the fields at the bottom of the form. Some of them do
not require entry by the user, but instead display or calculate
values. Let's look at these fields:
- Total Fee: This field totals the charges entered in the
above 8 lines of medical services. No user entry.
- Discount % and Fee: The discount % is shown if there is an
entry in the patient Master file specifying a discount. The
resulting discounted fee is also shown. If there is no
discount in the Master file, the discount % field is left
blank and the discounted fee is the same as the total fee.
No user entry for these fields.

- Prior Balance: Displays patient's prior balance, if any. No
user entry.

This field only comes up only when you are completing a new
Encounter Form for a patient. It is removed if you are
calling up a record to review or modify. This is because you
are usually concerned about the prior balance in the account
at the present time.
For instance, if you are reviewing an Encounter Form from 6
months ago, it is not terribly meaningful to know what the
prior balance was at that time. You would, of course, be
interested in the prior balance at the present time: this you
can find in the table listing the Encounter Forms for the
patient (or in the patient Ledger Report to be discussed
later). So when you pull up a prior Encounter Form for
review, only charges for that encounter are shown as well as
any payments or adjustments.

- Amount Due: This field calculates the Total Fee (or
discounted fee, if applicable) from the present Encounter
Form + the Prior Balance. No user entry.
*************************************************************
This field also only comes up when you are completing a new
Encounter Form for a patient. See discussion above under
Prior Balance.
*************************************************************
- Amount Paid: This field allows for user entry of payment
collected at the time of visit. The payment may be less than
the total fee or more if there is a prior balance.
If completing an Encounter Form for hospital services, you
would have a zero entry here (press the Enter_key).
- ROA: ROA stands for "received on account". We have adopted
the convention to have ROA refer to payments that are made
NOT at the office visit. This would include payments received
in the mail from the family or insurance payments. These
would be entered on a Posting Form (to be discussed later).
No user entry is required. The ROA is shown on the Encounter
Form for informational purposes. The distinction between
Amount Paid and ROA is admittedly somewhat arbitrary. Payment
made at an office visit toward prior services is entered
under Amount Paid, whereas payment mailed in for prior
services is entered under ROA (on a Posting Form). More about
this later.

- Adjustments: This field allows you to enter an adjustment
which will be deducted from the charges on the Encounter
Form. Certain managed care or insurance plans may require you
to adjust patient charges. If there is no adjustment,
press the Enter_key to go to the next field.
- Balance: The Balance field calculates the Encounter Form.
For a new Encounter Form, it is the sum of Total (or
Discounted) Fee and Prior Balance minus the sum of Amount
Paid and any Adjustments.
When viewing a prior Encounter Form, it is the Total (or
Discounted) Fee minus the sum of Amount Paid, ROA, and
Adjustments.
After reviewing the Balance, press the Enter_key to accept
the form; the Esc_key to go to previous fields to edit the
form; or Ctrl_Esc to completely abort the form.

KEY POINTS: ENCOUNTER FORM: Be sure you understand the following
concepts regarding the Encounter Form:
- The Encounter Number.
- Difference between Report Date and Date of Service.
- Entering Diagnosis and Diagnosis Code via the F9_key.
- Entering CPT Codes, Descriptions and Charges via the F10_key.
- The different financial fields.

<< LEDGERS and STATEMENTS >>
We will now review Ledgers and Statements.
We will pretty much use the terms Ledger and Statement
interchangeably. Technically a Ledger is a listing of a patient's
account history, whereas a statement is an invoice given to the
patient for billing purposes.
Make the selection from the Main Menu to get the Ledger and
Statement submenu. This submenu contains the following options:
- Display/Print Individual Patient Ledgers: This gives you a
table listing all patients in the database. Highlight the
desired patient to review her or his ledger. Note that you
can print the ledger by pressing the F3_key.
