  73. Or from Windows, launch Notepad and open the file Order.frm in the VSRC 
  74. directory/folder.  From the File menu, select Page Setup to set all margins
  75. minimally:  0 for Left, Right, Top, Bottom margins.  Delete the default 
  76. header and footer items.  Select OK.  Then from the File Menu, select Print
  77. to print out the order form.  (If you wish to fill in the form with your 
  78. order information before printing, it would be best to open Order.frm in a 
  79. word processor, such as Write/WordPad, since Notepad does not have an 
  80. overwrite mode.)

  82. You may also order with a note or letter that includes your name, organiza-
  83. tion name, address, program name & version, number of users being licensed,
  84. and remittance.  We would appreciate knowing where you obtained your copy
  85. of this program; we utilize this information on your source for future
