  1. Application for Authenticity Verification

  3. In order to receive a serial number to use with the PUTAV program, please 
  4. return the registration card that comes with the licensed copy of
  5. PKZIP.  Shareware Publishing will automatically send you your
  6. personal AV number based on the name entered on registration card.
  7.  
  8. At a later date you may wish to change your AV details.  For example
  9. if you originally used the program in the office and are now using
  10. it at home.  You can use this AV application form to request a
  11. change of AV code.  Just mail or FAX the form to Shareware
  12. Publishing. The address/FAX number is in your user guide.

  14. Polite Note: The provision of the AV code is a free service but it
  15. does take staff time.  If you request frequent changes to your AV
  16. code then we do reserve the right to make a small administrative
  17. charge to cover our additional staff time.

  20. Please note that this does not constitute a license from PKWARE or
  21. Shareware Publishing to distribute the executable programs PKZIP,
  22. PKUNZIP or self-extracting PKSFX files for commercial purposes.  See
  23. LICENSE.DOC or contact PKWARE Inc.  for more information about
  24. distribution licenses for the software.

  60. provide detection of tampered, hacked or virus infected files, we ask
  61. that you provide only your company's name or individual's name for the
  62. AV string.

  64. The "AVEXTRA.TXT" feature was designed to additionally display telephone
  65. numbers, addresses, BBS numbers, BBS nodes or parameter, CompuServe
  66. addresses and similar information.
