  70.   A. ORDER FORM
  71.      Use the number from the LEFT side of the catalog page to identify your
  72.      selection (some of our catalogs have manufacturer's ID numbers on the
  73.      right side of the page; these are not our catalog numbers).  Indicate
  74.      the title or a description of your selection and then the amount.
  75.  
  76.      On the title page of each catalog is an identifying number such as
  77.      "Cat. # 9121kba", please write this number in the place provided on the
  78.      order form.  Add shipping and handling (see below).  New Jersey residents
  79.      add 7% sales tax.  Total your order on the last line.  Indicate the
  80.      payment method you prefer;  Money Order or check.
  81.  
  82.      If you are paying by check, please put your signature in the place
  83.      provided. Clearly print or type your name and shipping address.  This will
  84.      be your shipping label.  Send you order to the address on the order form.

  86.   B. PAYMENT
  87.      Payments for your order can be made by Money Order or check.  Purchasers
  88.      paying by personal check must sign the order form where indicated,
  89.      aggreeing to reimburse the All Night Book Store for penalties incurred
  90.      as a result of checks refused by the bank.

  94.   C. SHIPPING
  95.      We will ship your order through the U.S Postal Service.  Add shipping
  96.      and handling charges in accordance with the following schedule:

  98.      Audio tapes:  $1.00 for the first tape and $0.50 for each additional.
  99.  
  100.      Video tapes:  $2.00 for the first tape and $1.00 for each additional.

  102.      We do not ship substitutes, if any item you have ordered is no longer
  103.      available, you will receive an immediate refund.
