  129. From windows program manager, select RUN and enter A:SETUP or B:SETUP.
  130. Follow the directions on the screen.
  131.  
  132. The setup program will install BOXES to your hard drive to the drive and
  133. directory you indicate.  It also will create a DynoTech folder and
  134. insert appropriate icons.

  137. DYNOTECH DEALER APPLICATION AND AGREEMENT
  138.  
  139. DynoTech Software ("DynoTech"), invites interested individuals and
  140. organizations to apply as independent distributors for the computer
  141. program "BOXES" (Shareware Edition). An accepted entity will be known as
  142. "DynoDealer."  A DynoDealer is an independent contractor, and is not an
  143. employee of DynoTech Software.  DynoDealers are responsible for all taxes
  144. and fees associated with their shareware distribution business.
  145.  
  146. For those accepted, here's the deal:
  147.  
  148. 1.  DynoTech agrees to pay a commission in US dollars equal to 5% of
  149. total registration payments received as a result of DynoDealer's
  150. verifiable distribution efforts.
  151.  
  152. 2.  DynoTech agrees to send DynoDealer a copy of BOXES containing a coded
  153. registration form on a 3 1/2" HD diskette, at no charge to DynoDealer.
  154. DynoTech will use the DynoDealer number shown on the form to verify
  155. DynoDealer's distribution efforts.
  156.  
  157. 3.  DynoTech agrees to send a check, drawn on a US bank, by the 20th of
  158. each month for all commissions earned during the previous month. However,
  159. DynoTech may not send a check until accumulated commissions exceed $10.
  160. Amounts equal to or less than $10 may continue to accumulate each month,
  161. until the total exceeds $10.  DynoTech will issue appropriate income tax
  162. forms and reports, as required, each calendar year (e.g. Form 1099-misc).

  164. 4.  DynoDealer agrees to register the "Champion Edition" of BOXES.
  165. DynoDealer's registration form must accompany this application, unless
  166. DynoDealer is already a registered owner of the "Champion Edition" of
  167. BOXES.
  168.  
  169. 5.  DynoDealer agrees to use all reasonably available methods to
  170. distribute BOXES.  DynoDealer must distribute all files on the BOXES
  171. diskette received from DynoTech.  Before distribution, DynoDealer may add
  172. one information file, but may not remove any files from the diskette.  If
  173. distributed on diskette, DynoDealer may sell the diskette for no more
  174. than $5.  If distributed in a compressed form, name the "zipped" file:
  175. BOXES.ZIP.
  176.  
  177. 6.  DynoDealer agrees not to distribute any registered editions of BOXES
  178. (includes any version or edition).
  179.  
  180. 7.  DynoDealer agrees that all disputes will be submitted in writing to
  181. President, DynoTech Software, 1105 Home Avenue, Waynesville, MO
  182. 65583-2231.  The President's decision is final.

  188. requested below.  Include, on a separate page, a legible summary of your
  189. background (resume is OK).  Also, tell why you would make a good

  191. after receiving your application.  If accepted, a copy of this form will
  192. be returned showing your DynoDealer number.
