  6. I, @001, desire to make aware that after mature reflection, and,
  7. being aware of the right under the law to decline life-
  8. sustaining treatment, that I wish, should I ever be unable to
  9. make decisions for myself concerning my medical treatment that 
  10. I receive life sustaining treatment even after a terminal 
  11. diagnosis, even if the life prolonging treatment will delay
  12. the natural process of dying.
