  3. This form is just for credit card orders for popular combo orders.  See
  4. the file ORDER.FRM if you want to order via check, cash, or money order
  5. or if you want to order a PMP Software product not listed here.

  7. If you have a valid Visa or MasterCard, you can register by calling
  8. NorthStar Solutions at 1-800-699-6395 from 10:00 a.m. to 10:00 p.m.,
  9. Eastern Standard Time.  NorthStar Solutions also accepts registrations
  10. via Compuserve (71561,2751) or the Internet (71561.2751@compuserve.com).

  13. information including signature to

  20. -The program, version number, and type of registration (1-12)
  21. -Where the latest version should be mailed
  22. -Your Visa or MasterCard # and its expiration date
  23. -Your disk size, 5.25 inch or 3.5 inch
  24. -Order code if known (on upper left of the order from, it's okay if
  25.  you don't know the code)
