  7. VITALI.EXE and VITALII.EXE are programs to tell you where to locate
  8. important records in the 50 States and District of Columbia.
  9.  
  10. The records include:

  12. 1.  Birth Certificates
  13.  
  14. 2.  Death Certificates
  15.  
  16. 3.  Marriage Licenses
  17.  
  18. 4.  Divorce Certificates
  19.  
  20. Just about everyone will be able to make good use of these
  21. programs.
  22.  
  23. Here is some information to include when requesting licenses or
  24. certificates:
  25.  
  26. Birth or Death Records:
  27.  
  28. 1.  Full name of person whose record is being requested.
  29. 2.  Sex.
  30. 3.  Parents' names, including maiden name of mother.
  31. 4.  Month, day, and year of birth or death.
  32. 5.  Place of birth or death (city of town, county, and State.  Name
  33.     hospital (if known).
  34. 6.  Purpose for which copy is needed.
  35. 7.  Relationship to person whose record is being requested.

  37. Marriage Record:
  38.  
  39. 1.  Full names of bride and groom.
  40. 2.  Month, day, and year of marriage.
  41. 3.  Place of marriage (city or town, county and State).
  42. 4.  Purpose for which copy is needed.
  43. 5.  Relationship to persons whose record is being requested.
  44.  
  45. Divorce Record:
  46.  
  47. 1.  Full names of husband and wife.
  48. 2.  Date of divorce or annulment.
  49. 3.  Place of divorce of annulment.
  50. 4.  Type of final decree.
  51. 5.  Purpose for which copy is needed.
  52. 6.  Relationship to persons whose record is being requested.

  55. different addresses and phone numbers.
